The type of health plan you have — PPO or HMO — affects which treatment centers you can use and how much freedom you have to choose. Here's what to know.

PPO plans

PPO (Preferred Provider Organization) plans offer the most flexibility. They typically let you see out-of-network providers and often cover treatment at private centers like Reform without a referral. Most people we work with have PPO plans.

HMO plans

HMO (Health Maintenance Organization) plans usually require you to stay in-network and get a referral from a primary care doctor. Coverage can be more limited, but treatment is still often available — it just takes a few more steps.
