Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment to Your Privacy

Reform Recovery Center (“Reform,” “we,” “us”) is required by law to maintain the privacy and security of your protected health information (“PHI”), to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of the Notice currently in effect.

Because we provide substance use disorder treatment, your records are also protected by the federal confidentiality rules at 42 C.F.R. Part 2, which generally prohibit us from acknowledging that you are a patient or disclosing any information identifying you as having a substance use disorder unless you consent in writing, the disclosure is allowed by court order, or the disclosure meets another limited exception permitted by law (such as medical emergencies, or reports of suspected child abuse and neglect).

How We May Use and Disclose Your Health Information

Treatment. We may use and share your PHI within our treatment team to provide, coordinate, and manage your care, and with other providers involved in your care with your written consent where required.
Payment. We may use and share your PHI to bill and collect payment from your health plan or other payers, with your written consent where required by 42 C.F.R. Part 2.
Health care operations. We may use PHI for quality improvement, training, licensing, audits, and business planning.
Appointment reminders and follow-up. With your consent, we may contact you by phone, email, or text about appointments and care.
As required by law. We will disclose PHI when federal, state, or local law requires it, including mandated reports of suspected child abuse or neglect, threats of serious harm, and lawful court orders meeting Part 2 requirements.
Medical emergencies. We may disclose necessary information to medical personnel in a genuine medical emergency.
Crimes on premises. We may report to law enforcement a crime committed on our premises or against our personnel.

Other uses and disclosures not described in this Notice — including most uses for marketing, any sale of PHI, and most sharing of psychotherapy notes — will be made only with your written authorization, which you may revoke at any time in writing (except to the extent we have already relied on it).

Your Rights Regarding Your Health Information

Right to inspect and copy. You may request an electronic or paper copy of your record. We will respond within 30 days and may charge a reasonable, cost-based fee.
Right to amend. You may ask us to correct information you believe is inaccurate or incomplete. We may deny the request in writing with an explanation.
Right to an accounting of disclosures. You may request a list of certain disclosures we have made of your PHI in the prior six years.
Right to request restrictions. You may ask us to limit what we use or share. We must agree if you pay for a service in full out-of-pocket and ask us not to share that information with your health plan.
Right to confidential communications. You may ask us to contact you in a specific way or at a specific location, and we will accommodate reasonable requests.
Right to a paper copy of this Notice. You may request a paper copy at any time, even if you agreed to receive it electronically.
Right to choose someone to act for you. A legal guardian or person with medical power of attorney may exercise your rights on your behalf.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue S.W., Washington, D.C. 20201, 1-877-696-6775, or online at www.hhs.gov/ocr/complaints. Violations of 42 C.F.R. Part 2 may also be reported consistent with federal law. We will never retaliate against you for filing a complaint.

Changes to This Notice

We may change this Notice at any time. The new Notice will apply to all PHI we maintain and will be posted on this page and available at our facility with an updated effective date.
