In accordance with California Senate Bill 349 (the “Ethical Treatment Covenant,” Health & Safety Code § 11833.4), Reform Recovery Center makes this Client Bill of Rights available to all clients and prospective clients. Every person who seeks or receives services from Reform Recovery has the right to:

How to File a Grievance or Complaint

Step 1 — Tell us. Raise any concern with your counselor or the Program Director, or contact us at hr@reformrecoverycenter.com or 818-590-5189. We will acknowledge your grievance promptly and respond in writing.

Step 2 — Contact the State. You may also file a complaint at any time, without first using our internal process, with the California Department of Health Care Services (DHCS), which licenses and certifies substance use disorder treatment programs:
